Registration Information


  • Send $100 deposit for each weekend, $200 for each 2 weeks of summer selected, or register online. A confirmation letter will be sent
    Click Here to register on-line. Online registration does require a brief initial set-up of your account with your first use 

  • No reservation made by phone will be held for more than 48 hours without a deposit.

  • Deposits are non-refundable and non-transferable. 

  • Complete paperwork as indicated on confirmation letter.

  • Paperwork must be returned by date indicated on confirmation or your camper will not be allowed to attend. 

  • If your camper has not attended camp within the past year, call ext. 111 and ask what forms are needed to update file, then follow above instructions.

  • DDS -Spaces for campers registering with a valid vendor service authorization from DDS must be  approved by Horizons and are limited.

  • You will receive a phone call once ALL paperwork has been submitted to complete the approval process.

  • Do not send any money until your camper is approved. Your check will be returned to you.

  • No space will be held until all paperwork is submitted.

  • Upon approval you must send us your deposit along with:

    • Medical Report Form

    • Medication Form 

    • Health History Update Form with a copy of Health Insurance card
